Player Forms (2009)

The following forms are needed for registration with NCR/USAV and with Keweenaw Select Volleyball:

Release of Liability (.pdf)

Tournament Contract Form (.pdf) must be completed and returned by Feb. 21

All forms must be completed and signed by players and parents in order for a player to participate in any tryout or practice. Please direct any questions to Club Director, Kaari Nevanen (kcnevane@mtu.edu)

Coaches' Forms (2009)

Event Travel
Everyone who was at the tournament and scrimmage (regardless of who drove) should fill out the event forms, one for the scrimmage and one for the tournaments in Duluth.  If you drove, enter your mileage at the bottom on personal car expenses. For the events put a daily per diem (M&IE) for the city you traveled to (i.e. Marquette, Duluth). National per diem rates


Travel Log
Also fill out a travel log for your practices, include date of practice, location of practice and miles driven. Please fill these out, print them off (in your print options, make sure you select fit to page so it doesn't span four pages!), sign them and return to either Joan or Carrie L. (both need to sign-off on these).
